What sets AFM apart is our in-office diabetes educator and dietitian. The providers at AFM want you to live your best and most healthy life and often send out referrals and orders to help our patients succeed.

Dr. Chelsea Ray, MD, founded Access Family Medicine. The ideas of independence and perseverance inspired her to open the doors. Access Family Medicine is a privately owned family medicine clinic.
