AFM offers a wide array of medical services in the office. From wellness visits, Medicare annuals, sports physicals, chronic care management, wound care, sick visits, and everything in between. What sets AFM apart is our in-office diabetes educator and dietitian. The providers at AFM want you to live your best and most healthy life and often send out referrals and orders to help our patients succeed.

Access Family Medicine Established 2021
Dr. Chelsea Ray, MD, founded Access Family Medicine. The ideas of independence and perseverance inspired her. In 2021, Dr. Ray opened Access Family Medicine with the help of Paula Ackerman MS, RD, CDCES, and Chelsea Texter NP-C, and since then, the trio has not looked back.
